Adam Balinsky is the Founder and President of Fifth Season Financial. He passionately believes in the mission of the company—to help relieve the financial pressures faced by many patients struggling with advanced illnesses including cancer.
Adam recognizes that people facing advanced-stage diseases have incremental financial pressures affecting everyday life that are not being addressed. That’s why Fifth Season’s financial assistance program, FLAG (Funds for Living and Giving), is designed to bridge the gap between the traditional financial assistance programs available to mitigate the direct medical and treatment costs, and the much broader financial needs of patients. The FLAG Program provides patients with money from a non-traditional asset – their existing life insurance policy.
Adam hopes that those who use our financial assistance program are able to use the funds to maintain or better their quality of life. The funds can be used as the patient sees fit. Common uses include paying rent or mortgage bills, utility bills, medication costs, treatment costs. Others use the funds to take a dream trip—creating lasting memories with family members, to pay for in-home care, or to pay to bring family together. To date, Fifth Season has assisted nearly 400 patients and their families and provided more than $75 million in financial help to patients with advanced illnesses.
The Story Behind Adam’s Passion for Fifth Season’s Services
A year after Adam and his family moved to the United States from Canada, his wife, Tracey, was diagnosed with breast cancer. While dealing with the sobering realities of their new situation, Adam began to learn about the many ways organizations in the U.S. support patients medically and emotionally. However, he was disappointed to see the very limited structured support for the broad range of financial strains patients often face. Adam was motivated and inspired to proactively do something about it. After serving as director of Fifth Season Financial from 2007 through 2009, Adam acquired Fifth Season Financial in October 2012. He then structured and developed the FLAG program to specifically address financial needs of people facing advanced-stage illnesses. Beyond Fifth Season, Adam and Tracey (who is now five years post treatment and doing very well!) are very active in supporting cancer causes, including Gilda’s Club Westchester, where Tracey is a member of the Board of Directors.
Adam’s Expertise
Adam is a lawyer by training, having practiced law for 11 years at Baker & McKenzie LLP, where he became a partner in 2005. Adam holds a joint MBA/JD from the University of Toronto and a Bachelors of Commerce from McGill University. He is admitted to practice law in the Province of Ontario, Canada, and is in good standing with the American Bar Association and the Law Society of Upper Canada. He has served on the Board of Directors of the Life Insurance Settlement Association (LISA) from May 2011 to May 2013.

ENT and Allergy Associates is celebrating its 20th anniversary since its inception. Congratulations! What are a few major successes the practice has had in this time frame in terms of growth?
What are the services that ENT and Allergy offers?
What are the benefits of ENT and Allergy, and how does your organization differ from other competing practices in the country in terms of physician care, technology, and so forth?
With October being "National Audiology Awareness Month" and "National Protect Your Hearing Month," what is one of the most important pieces of information that consumers should know regarding this topic?
ENT and Allergy sees more than 80,000 patients per month and is expanding its geographic footprint in the Tri-State area. Can you tell us a little bit about these expansion plans/goals?
Mr. Glazer is responsible for the overall operation for ENT and Allergy Assoc., LLP and focuses his attention on physician recruitment, mergers and acquisitions and strategic planning of the organization. Mr. Glazer is an ad-hoc member of all Board Committees and reports directly to the President of the group. ENTA consists of 200 MDs with 43 locations in the NY/NJ metro region. Mr. Glazer joined ENT and Allergy Associates (ENTA) in December, 1997 and has over Forty (40) years of healthcare administrative experience in finance and operations, information technology, managed care contracting, mergers and acquisitions, physician recruitment and healthcare marketing. OASIS – Otolaryngology/Allergy Specialty Insurance Services , a self funded self insured Medical Malpractice Captive was formed by the partners at ENTA in 2008, Mr. Glazer serves as the Vice President/ Secretary of this Vermont domiciled insurance company. He has served on the Leadership Council as President - Association of Otolaryngology Administrators (AOA) and has participated on ad-hoc committees of the American Academy of Otolaryngology – Head and Neck Surgery (AAO-HNS). He is a Board Member of the Westchester County Association – a fortune 500 business think tank. Mr. Glazer is an Adjunct Professor at Pace University Graduate Program for Public Administration - Health Policy. In 2015, Fordham University appointed Mr. Glazer as Program Coordinator of their Healthcare Career Workshop Program. He is also on the visiting faculty for Iona College. In 2016, he was re- appointed to the NYS Ninth Judicial District BAR Grievance Committee for a four year term.
Prior to joining ENTA, Mr. Glazer worked as a hospital administrator at New York University Medical Center (1976-1985) where he was a Departmental Controller, Columbia Presbyterian Medical Center (1985-1993) where he served as the Treasurer, New York United Medical Center (1993-1996) where he was the Senior Director of Finance and Business Development. Mr. Glazer transitioned to physician practice management in 1996 where he was the Vice President of Administration for ProHealth Associates, a multi-specialty sports medicine practice in Long Island. Mr. Glazer has a BS in Business Administration from the State University of New York and a Masters in Public Administration – Health Care-New York University.

Dr. Fiona Rahbar is a board-certified dermatologist by the American Academy of Dermatology, as well as a Fellow of the American Academy of Dermatology.
Dr. Rahbar has completed an undergraduate degree in Neuroscience and Behavioral Biology from Emory University. Subsequently she studied at and graduated from Vanderbilt University School of Medicine. She completed her dermatology residency at the University of Virginia, where she served as Chief Resident during her last year of training.
Dr. Rahbar has presented at medical conferences including the Society of General Internal Medicine and the Cosmetic Surgery Forum, and published in journals such as OurHealth, Internal Medicine Consultant, Dermatologic Surgery and Cutis.
In addition to treating patients in the U.S., Dr. Rahbar has volunteered in Haiti and Uganda, and has spent time doing HIV research in Buenos Aires, Argentina. She finds ways to volunteer when possible, including with the International Refugee Committee, Charlottesville Free Clinic and the Remote Access Medicine Clinic in Wise, West Virginia.

James LaValle, is a nationally recognized clinical pharmacist, author, board certified clinical nutritionist, founder of Metabolic Code Enterprises, Inc., a web platform and practice solution enterprise, launching AIR Support and the Metabolic Code Assessment. In addition, he founded an Practices at Progressive Medical Center in Orange County, CA. In 2001 he founded and operated LaValle Metabolic Institute, an interdisciplinary medicine facility in Cincinnati for the past 15 years (sold in 2014) where he served thousands of patients using his metabolic model for health. He also founded Integrative Health Resources 15 years ago, which is focused as a natural products industry consulting company. James has 27 years’ experience integrating natural therapies into various medical and business models. Dr. Charles Price is a pediatric orthopaedic surgeon at the Arnold Palmer Hospital for Children in Orlando, Florida and director of the International Hip Dysplasia Institute which he established with the help of comedian Larry the Cable Guy in 2009.
A past president of the Pediatric Orthopaedic Society of North America (POSNA) and the Limb Lengthening and Reconstruction Society of North (ASAMI), Dr. Price was chairman for 10 years of the International Pediatric Orthopaedic Symposium which he formed in 1998. He was instrumental in developing the muscularskeletal module for the University of Central Florida College of Medicine.
Dr. Price has contributed to the advancement of pediatric orthopaedics in the areas of limb lengthening, limb deformity correction, non-operative and operative management of scoliosis, and hip disorders in children. Dr. Charles Brenner Dr. Charles Brenner is the Roy J. Carver Chair and Head of Biochemistry at the University of Iowa, as well as a founding co-director of the University of Iowa Obesity Initiative. In 2004, Brenner, then a faculty member at Dartmouth College, discovered nicotinamide riboside (NR) to be a vital precursor of nicotinamide adenine dinucleotide (NAD+), which is made available by nicotinamide riboside kinases (Nrks) that are conserved between yeast and humans. In 2007, Dr. Brenner's lab discovered a second pathway by which NR is converted to NAD+ and showed that NR can extend the lifespan of yeast cells by virtue of elevating NAD+ levels and increasing the activity of the NAD+ dependent Sir2 enzyme.
In the past decade, Dr. Brenner has made multiple seminal contributions to NAD+ metabolism, which include engineering a yeast strain to convert inexpensive NAD+ precursor vitamins into NR, solving the crystal structure of human Nrk1, developing the methods for quantitative NAD+ metabolomics, and demonstrating the activity of oral NR in animal models of fatty liver disease, obesity, type 2 diabetes, diabetic and chemotherapeutic neuropathy, heart failure, and central brain injury. Dr. Brenner also led the first clinical trial of NR, which established safe oral availability in humans. He is currently focusing on the maternal and neonatal effects of oral NR and translating animal discoveries into evidence-based safe, human clinical practice for human conditions of metabolic stress.
